Webhooks and Discord

Platform ==> Raspberry Pi 3
NEMS Version ==> 1.5.2
I've been trying to get notifications sent to my Discord server by way of webhook but am completely unsure of how to go about doing it.
If anyone has managed this, can you give me some assistance or point me in the general direction of documentation (that I've probably missed).
